Clinical Practice Contact Report
ITP program form, Fall semester observation report
Responsibility: University Supervisor, in conjunction with Credential Candidate

Clinical Practice Expectations Form
ITP program form, Fall and Spring semester, includes dates, commitments, planning process, agreements discussed and completed at first Triad meeting semesters
Responsibility: Triad: Credential Candidate, Mentor Teacher, University Supervisor

Clinical Supervision Report
ITP program form, Spring semester report, student must submit a lesson plan for feedback before observation and meet with supervisor for feedback after observation
Responsibility: University Supervisor, in conjunction with Credential Candidate

Fall Clinical Practice Assessment (mid-term & final)
ITP program form, Fall semester, completed at mid semester and at the end of the semester, identifies strengths and areas for growth
Responsibility: Triad: Credential Candidate, Mentor Teacher, University Supervisor

Fieldwork Placement Verification
ITP program form, Spring semester Junior year, confirms the year-long placement
Responsibility: Credential Candidate and Mentor Teacher

Individual Development Plan
ITP program form, Spring semester, candidates must provide this to their induction program.
Responsibility: Triad: Credential Candidate, Mentor Teacher, University Supervisor

Junior Evaluation of Fieldwork
LSEE program form, Fall and Spring semesters, evaluation form used during junior year fieldwork placements
Responsibility: Professor and Cooperating Teacher

Performance Improvement Plan
ITP program form, used when expectations are not being met by candidate performance, makes a plan for improvement by a specific deadline, failure to improve may result in program dismissal
Responsibility: Triad: Credential Candidate, Mentor Teacher, University Supervisor

Request to Substitute Teach
ITP program form, completed in advance of substitute teaching request (See Handbook for more details)
Responsibility: Credential Candidate and Program Coordinator

Spring VELR Reflection
ITP program form, Spring semester, a coaching conversation and guided reflection of the VELR video and initial report
Responsibility: Credential Candidate and University Supervisor

Student Teaching Assessment - Final (mandatory)
ITP program form, Spring semester, completed at the end of the student teaching clinical practice
Responsibility: Triad: Credential Candidate, Mentor Teacher, University Supervisor

Student Teaching Assessment - Mid-term (optional)

Video Elicited Lesson Reflection (VELR) Form (provided by supervisor)
